H-form along with c-form application upload error (mvat)

Dear Members,

While uploading H-Form and C-form both in single application the following error occurs:

Duplicate Invoice Upload by this dealer --> 27......... --> 27......... --> H-FORM --> 20140701 --> 20140930 --> M-0022

anyone having knowledge of above query, please reply as our H-Form application is pending since 2014-15
