Yes , but I don't think it's available on portal yet

New form available & it ask for Detail of Purchase which not required in Q2&3

yes it's available , offline tool 4, 2.1 version

Up to December 2017, the details were not required in the Portal to enter. Q.E. 31.3.2018 to see
