Can anyone please tell me what all the documents that needs to be submitted along with the ARN Receipt to the Jurisdiction office

AND I SUBMITTED FOR THE M/O AUGUST SEPT AND OCT 2017
AND ALL THE DOCUMENTS IN THE M/O FEB 2018
BUT OFFICER SAID WITHIN 3-4 WEEK , WE WILL ISSUE THE REFUND

they are asking for the manual filled RFD 01A where can i get that...
