If activity is the supply of renting of motor cab then GST rate is 5% with NO ITC.

Salaried person is not supplying any kind of service, therefore, no GST.

let's say, the employer has hired cars for transporting its employees and is now required to pay service tax on the recoveries made. How does the employer determine the value of services attributable to each employee and accordingly raise an invoice?

no gst applicable on the perquisites given by an employer to the employee.
