Dear Suresh
All existing VAT / Service Tax / Central Excise Assessees are required to register their existing registration to new GST Portal. Every dealer will receive a provisional ID through which the migration has to be processed. This is compulsary for all the registered assessee under VAT / Service Tax / Central Excise then only on implemention of GST they wil be able to file the returns in the new GST portal.
The process of migration is explained in the website also.

Have the Government announced the sectors which will be exempted?
Like the Hospital Industry was exempted from service tax earlier. So should the hospitals and clinical establishments get registered under GST now?

The details of sectors which are exempted is not yet been notified. We have to wait till the final bill is passed in the parliment.
