Got notice from central tax office - bengaluru - wants to understand

Hi Guys, We got one notice from Supritendent Commissioner of Central tax - Bangalore. The notice is attached. Q. no. 1) I am looking for a clarification on what they are looking for? Q. no. 2) Got this letter through via post. How to reply them?
