Whether we need to show CGST and SGST amount in delivery challan and e-way bill while sending goods to job worker and also send me the format of delivery challan.

Yes u should mention all the things as we mention in tax invoice except we written it is delivery challan and other conditions we mention in tax invoice

There is no specific format for it but it should have all details as specified in the rules.
