Well adarsh is right as it is indeed a serious matter. But as per my experience there is no need to worry. You should first visit the RD office with the copy of following documents and have a chat with RD people regarding the procedure of fee adjustment:
1. Certified copy of rejected form-5.
2. Certified copy of paid challan.
3. Proof of payment of stamp duty.
4. Always kept copy of challan to refer SRN no.
There is no doubt about the adjustment of fee paid earlier and no need to pay fee again. Also visit ROC office simultaneously and have a chat with ROC people also. In your case you can not do anything without the help of administrative people sitting at the RD and ROC office. 100% probability of visiting MCA office at Shastri Bhawan, New Delhi also involved in your case. I think this is gonna be a tuff but a valuable experience for you.
