Is it necessary to attach notice or resolution with form-5 or just altered MOA is enough??please suggest

Altered MOA is enough...
In the form it is not asking the notice and explanatory statement as mandatory attachment.
So pls proceed with altered MOA.
But isn't it is under good practice to attach it.
it is good to attach.but not mandatory.
