While filling up Form 49B Tan Application, which date should be mentioned in "Item no - 7"?

can u specify what it asks coz when i am viewing the form it mentions only 6 items.

Dear Akshata
I'm using manual form only and not the online one.
That 7th item appears only in the manual form.
