Form 16

Hi,

I was employed last year only for a period of 6 months and my total salary income during that time was Rs. 143,928. After this period I became a consultant. The company I was working for has not issued any Form 16 for this period. They have not deducted TDS as well. My question is:  Should I show this as salary income in ITR 4? I have the salary breakdown for conveyance allowance and HRA.
