Form 108 query

I completed my 3-1/2 yrs articleship n submitted form 108 on 22nd feb, but then i realised that in weeks column i filled details of just 3 yrs as it indicated year1, year2 and year3. So, now what should i do for correction of the same?? and will my articleship be extended??? plz reply fast..m very much tensed and i submitted form at Vishwas nagar
