FORM 107, HOW TO FILL UP?

Please help me out in filling FORM 107.....? i have already submitted FORM 108 by declaring excess leave, without considering FORM 107...now i have got letter from the chennai asking for the FORM 107....., should i execute FORM 107 now....., if yes.... please help me out in filling.& what date the stamp should be....? Thanks in advance

Can anyone please tell me the process or give Dummy 107 filled  form. I had completed my Articleship on 15 th of August and had to continue with same partner as he had written an excess leave of 7.5 months. Please mail me on aniket7742 @ gmail.com
