Please download and submit the form which is meant for offline purpose, I have attached the form also with this answer, further to answer your question, no you don't need to pay anything for re-registration as also mentioned on page 12 of the form, payment is only to be made when first time registration is done with Form 103, the same is also mention on page 11.
