You may fill up the form online(icaiexam.icai.org) and pay fees by debit/credit card and post it to icai or collect physical form institute office by paying rs.500.

Yaar tu bhi khyalo mai dube ho. icai jao form purchase mai 500 rs waste karo. yahi treatment thik rahega my dear freind.
