Fema query

I have a query....this is regarding fema...... I have indian currency amounts to RS. 68 lacs approx in the form of FD... i need to carry out this money from india.. i have a PR of australia... foreign currency amounts $ 145000 approx... can u plz tell me.. what is the procedure.. what are the forms requires to be filed.....
