Extension of period

Hi

We had executed an agreement with another company for an initial period of 5 years to be extended further for a period to be mutually decided by both the parties. Now both parties have decided to extend the period for further 5 years on the same terms and conditions as per earlier agreement. How to write this legally? Will an addendum ok in this case?
