Forms will be available in the Regional and Branch Offices of the Institute on or from 6th February, 2012.

As per instructions given for OMR Application Form it says that name of the candidate should be as recorded in registration letter issued by ICAI. So, i don't think its possible for you to change the spelling of your name. For any further possibilities, contact institute or e-Sahaayataa
