Error in C Form, urgent !!!

Dear All, Plz help me out.?

We hav two companies A and B, while filling the details in C form issued by department, by mistake details of B ltd is filled in in form issued to A ltd.

now wats the solution, can we use whitener in the formand continue or hav to file againg for C form.??
