Employer did not include my name in form 24q

I received Salary from Three employers in Financial Year 2018-19. From one employer for example around 570000.00 and he has issued Form 16 A and also my name included in Form 24Q. Second employer paid for one month i.e April 2018 around 19000.00 and issued Form 16 A and included my Name in Form 24Q even though, he has not deducted any income tax. Third employer paid for March 2019 around 21000.00 has not deducted any income tax and did not issue Form 16 A ( issued Form 16 Part B) and also did not include my name in Form 24Q. How to file return? If I include 21000 then there will be a mismatch with the information available with Income Tax Deptt and If I hide then it will be against law. Please advise. Note, the First employer has already deducted more income tax and even if include 21000, I will still get refund, but what about mismatch.
