form 23 B is the form
You have to fill form 23B which is submitted to ROC electronically.
You can download form 23B from MCA21's office website. link is given below:

Every Auditor/ Auditor Firm of the company is requiring giving intimation of his/ its appointment/ re-appointment to the ROC by filing FORM-23B ON-LINE. before filing of BALANCE SHEET. It is important to be noted here that FORM-23B has to be digitally signed by AUDITOR only. All Auditor/ Auditor Firms are now requiring having Digital Signatures for filing of e FORM-23B.
23B was for FY 13-14.
Now for t current year FY 14-15, ADT 1 has to be filed in GNL 2 within 15 days from AGM. A letter also has to be obtained from t auditor that he is not disqualified from being appointed as t auditor of t company in terms of section 139 & 141.
