Dsc problem with mca forms

I am continuosly having this problem on attaching DSCs to MCA forms. Please help.

uninstal acrobat and install adobe reader 11.0.10 from below link

Tried with adobe 11 as well but the problem persists. Tried on a different system but no use.

first click on check form -> affix dsc -> click on prescrutiny then upload

check form is OK, errors pop up when I start attaching DSC and then prescrunity does not happen, but if I press prescrunity again it says that it has already been done but the button stays active.

no? I have only turned off signing revocation status to reduce pdf size...Is anything else required?
