Download of exam application form

Hi,

I heard that cs executive examination form can be downloaded.can anyone please send me the link.

Based on my knowledge I have to take DD for this much amount,right?

1200+1200=2400 (for both modules)

+500 (for application form) ie a TOTAL OF RS.2900.

Correct me if I am wrong?
