Doubt regarding payment made to transporters

I am having a query regarding TDS for transporters..

As we all know " Deductors who make payments to transporters without deducting TDS( as they have quoted PAN) will be required to intimate these PAN details to Income Tax Department In the PRESCRIBED FORMAT..

So i want to know the "prescribed format" in which we have to make an intimation to Income Tax Department.
