hi , i am having medical aiiowance in my payslip . it is fully taxable . can i claim my medical exp as per 80d

No, 80D is only for medical insurance premium not expenses.

i am having medical allowance in my payslip of Rs 15000 /- per year from empolyer. i am incurred RS 5000/- for preventive health checkup . In these case , can i claim it under sec 80D
Yes, you can claim it even if you don't get medical allowance. The two are not connected in any way.
