One of my client (service holder) is differently abled. His certificate shows 71% Disability. To avail the Deduction U/S 80 U, what documents should I collect and upload.

To my knowledge there is a health certificate which is certified by the recognised hospitals, kindly make that available.
Form 10-IA duly signed by medical not below post graduate qualification or any other medical board.
1. Should I have to upload it or kept it with us ?. 2. Should one have to collect it for each f.y. ?
