Affidavit format for inc32 spice form for co incorporation

Can someone please provide me with the format of "Affidavit and declaration by first subscriber(s) and director(s)" as required in new company registration form spice, INC 32 .

and what should be the value of the stamp paper for such affidavit ?

format like INC 9 and no stamp duty required.
It can attach in plane paper also..

as per companies amendment act , 2017 subscribers and frst directors have to give a declaration and not affadavit

I........ , being the subscriber to the memorandum /named as first directors of above name proposed company , hereby solemnly declare amd affirm that :
1. I have not been convicted of any offence in connection with promotion , formation , or management of any company during preceding 5 years ; and
2. I have not been found guilty of any fraud , misfeasance or of any breach of duty to any company under this Act or under any previous company law dyrung preceding 5 years ;and
3, All the documents filed with registrar for registration of the company contain information that is correct and complete and true to the best of my knowledge and belief
