& In statement there is no IFSC code mentioning what can I do for the further things?
may I upload the letter regarding about this issue from a branch manager with seal and signature

You can upload Your Pass Book Front Page...
Visit your bank, ask for a single cheque leaf. They will do that. Later you may upload this cheque containing account number and IFSC.
