Cs response sheet

Hi,

                  I am undergoing oral coaching from chapter.But when I asked our sir he said we have to submit the response sheets.So what are the procedures.......like anything we have to send along with response sheets and what are the things we should take care while answering ,can we use A4 size sheets and finally the ADDRESS?
