Can anyone tell me from where will I get the examination application form for this December and how much fees I have to pay considering that I am eligible for one module only?Please tell me the complete procedure?

2 If you already have an account, just login , if you are not having an account on this site click reset password and login
3 Now click Exam enrollment tab
4 a form appears
5 fill up the form
6 make online payment
