you can fill up the application online, and print the same and submit it to your range office along with PAN and incorporation certificate

Log into ACES and amend ST1 and submit the hard copy of amended ST1 along with the proof of name change.

Go to www.aces.gov.in
Logon to your account and click "Amend ST1"
Take the print out of amended ST1 and submit to the respective jurisdiction.
You will be receiving the ST 2 in few days through mail

Procedures:
1st step - Log in to Service tax website- (www.aces.gov.in)
2nd step-Fill form ST1 Amendment details.
3rd Step-Online Submision of form ST 1.
4th Step - Submit hard copy to the department with proof.

In case of Privat Company while filling the Form ST-1 online, what should be mentioned in Name of applicant, whether the name of company or its director appying fro Reg.,pls. guide
