Change in surname
I am about to change my current surname to my actuall family name. From where to get afidavit? Is it a stamp paper or typed on blank paper? On what conditions does notary attests such affidavit? Who are the witnesses? What's the further procedure?

Law will not permit the change of surname. If any spelling correction, it can be done through gazette notification by applying in the gagette department.
