Certificate of service. urgent !!!

Dear Friends,

Alongwith CA final exam form, ICAI has given a certificate of service. I got my principal's signature in that form as well as in the exam application form.

Apart from this do i need to submit a 'Certificate of service' document signed by my principal in my firm's letterhead?

Pls clarify!
