Certificate from practicing professional

Dear All,

 A Pvt. LTd. Company wants to make outward remittance to pay interest on compulsory convertible debentures to its debenture holders.for this Authorised Dealer wants a certificate from a Practicing Professional confirming the total number of CCD issued. Can anyone please send me the format or draft of the same?
