Can't fill up from by debit card due to forgot 3d paswrd

Previously I filled on line exam form form by using 3D secure one tym pswrd of my debid card. Nw I cant fill up on line exam from because of forgottn my One tym 3D secure pswrd. Now how cant I get this paswrd or how can I fill up on line exam form by using my debid card???? I need ir valuable suggestion..
