My supplier sent back 3 years old "C" form for correction, but our karnataka commercial tax asking affidavit on stamp paper and original, duplicate and conterfoil copy.
Pls send me the formate of affidavit.

It is not prescribed in the act you have to provide the same to the satisfaction of the assisting authorities
