C.a final registration...

I recently cleared PCC in november 2011 both groups,my articleship ended on 13/03/2011,please tell me the procedure for final registartion,

1.can i take print out and fill final registartion form downloaded from icai website?

2.which forms are to be attached?

3.DD should be in whose favour(i want complete address)

4.can i give c.a final in november 2012?

5.DD should be of how much amount?
