Articleship completion procedure

I have just completed my articleship. Plz let me know the procedure to be followed.

 

1. Both 108 and 119 are to be filed or only 108?

2. I have collect to these forms from the branch office or I can download them?

3. I have submit them at the branch or post them?

 

Please answer quickly. I hav to proceed with it....

Procedure is given below. First & foremost, you need to submit Form 119 to your principal (employer). Form 119 is a Requisition Form for issue of service certificate. Based on this, your employer will give you Form 108 which is a Certificate of Services under Articles. You need to send Form 108 signed by your employer to the institute / council within 30 days of completion of your articles. If you send after 30 days then you need to pay Rs. 100 as late fees. Delay beyond 30 days upto 180 days requires payment of Rs. 300 as late fees. Late submission fees slabs are mentioned on Form 108. Download form 119 from ICAI. Form 108 you should get from council or offices of ICAI. In short send original signed form 108 (signed by employer), acknowledged copy of form 119 & a covering letter to ICAI within 30 days of completion of articles.

I had done my articleship under 2 principals , this is my 3rd principle. Is the details of my present principal is needed for my completion of articleship
