Applicant name - service tax registration

Dear Members, In the case of registration of partnership firm of the Service Tax, in Form ST-2, the name which is appearing is of one of the partners name and not of partnership firm. I want to know whether this is correct, if not what is the procedure to amend this certificate and what supporting documents will be required to be produced. Thank you.
