Can anyone suggest if there is any letter format to claim grounds before CIT(A) in income Tax ?

There is no specif format. You will have to write a letter to CIT (A) and claim the additional grounds during the proceedings of appeal.
However, I suppose it is the descrition of CIT (A) to allow the additional ground or not.
