Hello my name is Shivangi Agrawal. I had registered with ICSI in March for CS foundation. They had registered me as Shivangi Agarwal instead of Agrawal. Please help me to rectify this mistake. What are the necessary process to modify this.

firstly u will contact the icsi institute
cont. no. delhi office 011-45341076-77.and noida office 0120-4522000
and ask the problem.
u can contact noida office(Director of student service) through letter as well as mail, and take DD for Rs.50 in favour of ICSI, New Delhi , then they will change ur name and issues new ID card

my was registured with ICSI as MAHARANA PRATAP CHAUDHARY , and its changed to VISHAL CHAUDHARY via an affidavit of an first class magistrate.
i pass cs foundation programme,in june 2013 batch, n now m pursuing for cs executive 2014 dec term.
so don't know what to do next. somebody please help me,
